- For parents of minors requesting to volunteer, please fill out the Volunteer Request Form for Minors below. You will be contacted by our Office Staff with more information once the request is approved. If your child is requesting to volunteer for service hours, print out the Service Hour Record Form and bring it with you to the event. The designated event coordinator will sign off on the service hour form. Please do not show up to volunteer for an event without prior approval. We thank you for your service and cooperation in keeping everyone in a safe environment!
